as for the reason why it's unlikely, it's becouse dragonslayer arrows suck. most dragons clear a dc 20 fort save with ease...
It's a juvenile red dragon, so it has +14 fort.
With a misfortune that's a 12. so on 8+ it saves, and only takes normal damage...
granted, if he used a greater dragonslaying arrow that puts him down to about 15% of save or die after debuffs is slightly stronger, but vasting 2282 to 4057 gp on a single use arrow that on a save otherwise works as a mere +1 arrow...
and if you go pathfinder, it's has been even heavier nerfed...
also: divination mages ability to save a dice throw is OP. we didn't kill the last runelord this way, but we thought about it... we did after all have his name...
although the dragon would have had to have failed the will save for the misfortune to take effect, which isn't that easy. easier than the dragonslayer, but not by much.
